Real-time Clinical Decision Support
• CSIRO-designed algorithm employed for Health Care Homes recruitment in Australian GP practices
• Real-time in-hospital decision support to reduce unplanned chronic disease rehospitalisation trialled in hospital and currently being developed for state-wide risk stratification
• FHIR based delivery of clinical decision support successfully trialled in NSW demonstrator project
• Data from Queensland’s integrated electronic Medical Record (ieMR) used to develop algorithms to predict patient risk of deterioration in near real time
Evaluation of Victorian HealthLinks Chronic Care Program
• Evaluating the effectiveness of flexible funding to reduce hospital readmissions for an at-risk group
• Ten Victorian health services participated in the trial
• At-risk group identified using a statistical model-based algorithm
• Mixed-methods design using the RE-AIM framework to measure outcomes
Helping health services respond to crisis and manage “Business as Unusual”
• Developing demand prediction and aberrance detection algorithms to support real time operational decision making at the Queensland Patient Access Coordination Hub (Q-PACH)
